This list is not all inclusive as new plans and changes to the old ones occur daily in the insurance industry. Please be sure to bring all your insurance cards with you at the time of your appointment. With the hundreds of insurance policies available and the different rules that apply to each, it is not possible for this office to guarantee payment by your insurance plan for every service provided. We will do what we can to help make the billing process as easy as possible. It is your responsibility to contact your insurance carrier before your first appointment to check the In Network, Out of Network, Point of Service or HMO status of your policy. Some insurance companies require Referral Numbers or Referral Forms prior to your appointment. Typically a toll free number is listed on your insurance card to help you with your questions.
